CPR Class Waiver and Release of Liability

For CPR Training Class

This Waiver and Release of Liability (“Agreement”) is entered into by the undersigned participant (“Participant”) in favor of the instructor(s), training organization, and any affiliated entities, officers, employees, volunteers, and agents (collectively, the “Released Parties”).

1. Acknowledgment of Risks

The Participant acknowledges that CPR training involves physical exertion and practice, including, but not limited to, chest compressions, rescue breathing, and the use of training equipment. These activities may pose risks of personal injury, illness, or discomfort, including but not limited to muscle strain, fatigue, physical contact, allergic reactions to equipment, or other unforeseen risks.

2. Assumption of Risk

The Participant voluntarily assumes all risks, known or unknown, associated with participation in the CPR training class. The Participant accepts full responsibility for any injury, loss, damage, or other consequence that may result from participation, whether caused by the negligence of the Released Parties or otherwise.

3. Release and Waiver

In consideration for being permitted to participate in the CPR training class, the Participant, on behalf of themselves and their heirs, assigns, representatives, and next of kin, hereby fully releases, waives, discharges, and covenants not to sue the Released Parties for any and all liability, claims, demands, actions, or causes of action arising out of or related to participation in the class, including, without limitation, any claims of negligence.

4. Medical Authorization

The Participant affirms that they are in sufficient physical condition to participate in the training. In the event of a medical emergency, the Participant authorizes the Released Parties to secure emergency medical care at their discretion and agrees to assume full financial responsibility for such care.

5. No Guarantee of Certification or Future Performance

The Participant understands that completion of the class does not guarantee certification or proficiency in the skills taught by the class. The Released Parties make no representation or guarantee regarding the Participant’s future ability to perform CPR or utilize skills obtained in the training.

6. Governing Law and Severability

This Agreement shall be governed by and construed under the laws of British Columbia, Canada. If any provision of this Agreement is held invalid or unenforceable, the remainder shall continue in full force and effect.

I HAVE READ THIS WAIVER AND RELEASE OF LIABILITY, FULLY UNDERSTAND ITS TERMS, AND SIGN IT FREELY AND VOLUNTARILY. I ACKNOWLEDGE THAT BY SIGNING THIS DOCUMENT, I AM GIVING UP SUBSTANTIAL LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE.
